Brain and Nerve Surgery Specialist Assoc. Dr. Özhan Merzuk Uçkun stated that many patients had their teeth extracted due to lightning-like pain and that the disease had more than one treatment method.

Trigeminal Neuralgia, which manifests itself as a result of compression of the trigeminal nerve in the brain for various reasons during its spreading to the facial area, is known in the society as “Delirious Disease” or “Suicidal Disease”. The disease, which occurs due to reasons such as compression of the cerebellum on the nerve, tumor compression, inflammation, multiple sclerosis, is seen in 5 out of 100 thousand people in the world.


Stating that the pain in trigeminal neuralgia is seen as sudden and attacks in the face area where this nerve is dispersed, Brain and Nerve Surgery Specialist Assoc. Dr. Özhan Merzuk Uçkun said, “It occurs in the upper and lower jaw, cheek and eye area, which are the distribution of the trigeminal nerve. The pain is throbbing and lightning. It comes and goes at regular intervals. As the day passes, pain intervals become shorter and unbearable. Pain is triggered in situations such as eating, shaving, brushing teeth, washing face. The patient can describe this pain as a toothache, and the first referrals are dentists, so diagnosis may take time. Sometimes the pain is severe enough to pull his teeth, “he said.


Stating that the treatment of trigeminal neuralgia varies according to the age of the patient and the underlying disease, Assoc. Dr. Özhan Merzuk Uçkun said, “If there is an underlying tumor or mass compression, surgery is recommended first. Apart from this, 3 types of treatment methods are applied in trigeminal neuralgia. Medication is the first choice in both diagnosis and treatment of trigeminal neuralgia. Treatment is started with a low dose. It is used in combination when necessary. Doses are increased according to the severity of pain. “If there is no response to pain at the maximum dose and combination or if the drug develops side effects, then interventional and surgical methods are applied.”


Stating that the interventional method is the most widely used method in the world, Assoc. Dr. Uçkun said, “The reason is that the method can be used safely in middle-aged and elderly patients and can be repeated when necessary. The essence of the application is based on the principle of controlled damage to the trigeminal nerve fibers with the needle-electrode system inserted between the two lip junctions. The surgical method, on the other hand, is based on the principle of removing this pressure if there is a vascular pressure where the trigeminal nerve comes out of the brain stem. While the early period success rate is 99 percent, the success rate varies between 63-84 percent for 5 years. When surgery is performed in experienced hands, the complication rate is very low. It can be easily applied to elderly patients. “It is based on the placement of Teflon material between the nerve and the vein without the condition of damaging the nerve.
