Gynecological Oncology Specialist Assoc. Dr. Veysel Şal gave information about the advantages of laparoscopic, robotic and hysteroscopic surgery in gynecological diseases.
Endoscopic surgery method in gynecological diseases has been actively applied since the 90s in Europe and especially after the 2000s in our country. In the first years, it was applied to certain patient groups, but today it can be applied to many patients. Endoscopic surgeries in gynecological diseases are called laparoscopic-robotic surgery and hysteroscopic surgery.
Laparoscopic-robotic surgery is effective in many diseases with risk.
Laparoscopic-robotic surgery is the application of the appropriate surgical procedure after opening 2-3 incisions on the navel and on the side of the navel, inflating the abdomen. Small incisions are made in the abdomen as a surgical scar. The main trace entered from the navel is 1 cm and there are 2 or 3 holes of 0.5 cm on the right and left. Many surgeries related to gynecological diseases such as uterine fibroids, cysts, adhesions in the abdomen, removal of the uterus, uterine cancer, sometimes cervical cancer, tube opening, tube removal can be performed endoscopically.
At the beginning of the operations performed with endoscopy, there are especially chocolate cyst and chocolate cyst disease. Chocolate cyst is a very important disease as it reduces the number of eggs in women and causes infertility. The gold treatment method in chocolate cyst removal and surgery for chocolate cyst disease is endoscopic surgery. Open surgery is only applied if there are serious problems such as masses that fill the entire abdomen. If the uterus is to be removed in laparoscopy-robotic surgery, it is usually removed from the natural cavity, namely vaginally. Structures such as fibroids and cysts are taken out of the body by reducing their size with certain techniques through small incisions. Myoma surgery performed with endoscopic method provides much higher comfort to the patient compared to open myoma surgery. There is much less risk of intra-abdominal adhesions. The fact that this risk is low is very important, especially in myoma surgeries. Laparoscopy or robotic surgery method requires 1 or 2 days hospital stay depending on the patient’s condition.
Hysteroscopic surgery increases success rate in IVF
Hysteroscopic surgery is the method of imaging the inside of the uterus with a light camera. Laparoscopy means viewing the abdominal cavity with a camera, while hysteroscopy means viewing the inside of the uterus with a camera. Hysteroscopy is a procedure performed vaginally while lying on a normal examination table, as if it were an examination, often under anesthesia. Look inside the uterine cavity. The presence of problems such as a piece of flesh called a polyp, a fibroid that has grown into the uterus, adhesions caused by a previous abortion in the uterus called Asherman’s syndrome, and a congenital curtain in the uterus are examined with hysteroscopy. In addition, hysteroscopy is recommended for families who decide to have IVF to increase the success rate by stimulating the cells in the uterus before the procedure. It is also applied to expand the uterine cavity. It is a daily operation. You are discharged from the hospital 2-3 hours after the half-hour procedure on the same day.
Laparoscopic-robotic and hysteroscopic surgical methods are the most preferred surgeries in gynecology and should be performed by experienced hands.
